Please download and complete all the forms below and return completed forms to any staff member, or email to Christine@phoenixvilleseniorcenter.org. If you have questions, please call us at 610-935-1515. Becoming a Senior Center Participant is free. Please note that we are unable to conduct tours of the Center during lunch service (Monday – Friday, 11:30am-12:45pm).
After we put your information in our computer, you will check in at the computer each time you come to the center.
